Hydrochlorothiazide makes the kidneys clear salt and water, which lowers blood pressure and relieves swelling, but it also loses potassium. Amiloride is added because it holds potassium back, so the pair works together without draining it.

Precautions

Blood tests for potassium, sodium and kidney function are important, since amiloride can push potassium too high while the thiazide pushes it down. Avoid potassium supplements and salt substitutes unless your doctor approves. Take it in the morning. Report muscle weakness, numbness, confusion or an irregular heartbeat.
